Vaccines are required for Wisconsin students* You can get a copy of the Student Immunization Record from your school or download a copy.
At the start of the new school year, give the complete Student Immunization Record to the school. You can track and print your student's vaccine record through the Wisconsin Immunization Registry and/or your healthcare provider.
NEW requirements beginning for the 24-25 school year. The purpose of these changes is to bring Wisconsin closer in line to the Advisory Committee on Immunization Practices nationwide recommendations and in line with neighboring states’ school requirements.

Note: Children must be up to date on all vaccines listed for previous grades. For example, if a 7th grader is missing a dose of hepatitis B, they’ll need a catch-up dose of hepatitis B prior to 7th grade matriculation.
*Vaccine requirements may be waived for health, religious, or personal conviction reasons. Completed recommended vaccinations and/or waivers need to be on file at the school.
**Exceptions to the varicella vaccine requirement will be allowed in both child care centers and schools only if the child has had a case diagnosed by a qualified health care provider.
